What documents do I need to bring?
Important: All documents you present for your proof of residential address must show the exact same address – including street number, apartment number, city, and zip code. Even small differences can cause your application to be rejected.

What if I don’t have address documents in my name?
If you cannot provide two approved Florida residential address documents in your name, but live with someone else (the certifier), you may use a Certification of Address, Form HSMV 71120 (PDF).
How does the Certification of Address form work?
The certifier (the person providing proof of address) must either:
- Accompany you to our office to sign the form in front of an examiner OR sign the form in front of a notary.
- Provide two documents proving their own address (one must be a deed/mortgage, property tax bill, recent monthly mortgage statement or a rental/lease agreement).
For minors: Parents or guardians must provide two proofs of address in their own name plus documents showing the family/guardian relationship. They do not need to complete the Certification of Address form.
