Before You Begin

Concealed Weapon License Number (Required for Both Replacements & Revisions)

Your concealed weapon license number is required to complete a replacement or revised license transaction. If you cannot locate your license number, contact the Public Inquiry Center at (850) 245-5691 for help.

Notarized Statement (Required for Replacements)

All replacement license requests must include a notarized statement confirming the license is lost, stolen, or destroyed.

How to Request a Replacement License?

You can request a replacement license through these methods:

  1. Make an appointment to visit our North Tampa office in person (notarization services are available).
  2. Complete your request through the Florida Department of Agriculture and Consumer Services (FDACS). You can either visit one of their regional offices or mail them your request. Learn how to to complete a replacement request with FDACS.

How to Request a License Revision?

Need to updated your name?

If your name has changed, you may request a revised license. You must provide a copy of a legal document as proof of the name change, such as:

  • State-issued driver’s license or ID
  • U.S. Passport
  • U.S. military ID (active/reserve/retired)

Important: Name change revisions cannot be printed at the Tax Collector’s office. These revisions must be reviewed by the Florida Department of Agriculture and Consumer Services (FDACS) and will be mailed from Tallahassee.

Need to update your contact information?

If you need to update only your contact information (address, phone number, or email address), a replacement license is not required. The easiest way to submit updated contact information is through the Florida Department of Agriculture and Consumer Services (FDACS) website.

Do you have incorrect information on your license?

If there is incorrect information on your license (such as a misspelled name or incorrect date of birth), contact Florida Department of Agriculture and Consumer Services (FDACS) directly.
