Renew Your Disabled Parking Permit
To renew a disabled parking permit, you must submit a new Application for Disabled Person Parking Permit, Form HSMV 83039 (PDF). Both you and your certifying medical provider must sign the application.
- You can find a list of approved certifying medical providers on the Application for Disabled Person Parking Permit, Form HSMV 83039 (PDF).
- The form is not valid for more than 12 months from the date of the certifying medical provider’s signature.
Cost: There is no fee for permanent disabled parking permits.
