Replace Your Disabled Parking Permit
To replace a lost or stolen disabled parking permit, you must complete the following forms:
- Application for Replacement License Plate, Validation Decal or Parking Permit, Form HSMV 83146 (PDF).
- Application for Disabled Person Parking Permit, Form HSMV 83039 (PDF). Both you and your certifying medical provider must sign the application.
- You can find a list of approved certifying medical providers on the Application for Disabled Person Parking Permit, Form HSMV 83039 (PDF).
- The form is not valid for more than 12 months from the date of the certifying medical provider’s signature.
