Replacement Plate or Decal

When can I get a replacement?

You can apply for a replacement if your license plate, validation decal, or parking permit has been:

  • Lost or stolen
  • Damaged or defaced
  • Destroyed
  • Lost in transit (applied for but never received)

Who can apply?

Only the registered owner or lessee can apply.

What do I need to bring?

  1. A completed replacement application form.
  2. One of the following identification numbers:
    • License plate number
    • Driver license number
    • Decal number
    • Vehicle identification number (VIN)
  3. One of the following valid photo IDs:
    • U.S. driver license or identification card issued by the Department of Highway Safety and Motor Vehicles
    • Passport. The passport must be unexpired and have one of the following:
      • A stamp or mark affixed by the United States Department of Homeland Security onto the passport to evidence and authorize lawful presence in the United States; OR
      • An unexpired I-94, or current permanent resident card, or unexpired immigrant visa, issued by the United States Department of Homeland Security.
    • Canadian driver license or identification card
  4. Additional requirements (depending on situation):
    • For stolen items:
      • A police report should be submitted with your application
      • With police report: No replacement fee required
      • Without police report: Replacement fee required
    • For lost, damaged or destroyed Items:
      • Replacement fee required
      • No police report needed

Once you have all of the items listed above you will need to visit one of our offices to submit your documents and pay applicable replacement fees. View replacement fees.
